Overhead Cable Triceps Extension
The overhead cable triceps extension is a standing exercise where you hold a rope behind your head with your arms overhead, then straighten your elbows against the pull of a cable. Training with the arms raised stretches the long head of the triceps, which makes up the largest part of the muscle, under constant cable tension.

What muscles does the overhead cable triceps extension work?
Primary movers
- Triceps brachii (long head)
Assisting muscles
- Triceps brachii (lateral and medial heads)
- Anterior deltoid
- Rectus abdominis
The long head of the triceps crosses the shoulder joint. When your arms are overhead it is stretched at the start of the rep, and a stretched muscle tends to be better stimulated.
A cable keeps tension from the stretch to the lockout, whereas a dumbbell is lightest at the bottom. This means the long head works through the whole range.
The core and shoulders keep your torso still and your elbows pointing forward. Leaning forward in a staggered stance gives a stable base against the pull.
The movement is limited by elbow and shoulder comfort. If your shoulders feel tight overhead, use a smaller range or the kneeling version.

Common overhead cable triceps extension mistakes
Flaring the elbows
Wide elbows reduce triceps tension and strain the shoulders.
The fix: Keep your elbows pointing forward and close to your head.
Arching the lower back
Letting the ribs flare to make the lift easier loads the spine.
The fix: Brace your abs and keep your ribs down.
Moving the upper arms
Swinging the elbows turns it into a press.
The fix: Keep your upper arms fixed and move only at the elbow.
Too heavy
Heavy loads pull you off balance and shorten the range.
The fix: Use a weight you can control for 10 to 15 reps.
How many sets and reps?
| Goal | Sets | Reps | Load | Rest |
|---|---|---|---|---|
| Hypertrophy | 3–4 | 10–15 | Moderate, 1–2 RIR | 60–90 s |
| Stretch focus | 3 | 12–15 | Moderate, slow lowering | 60 s |
| Beginner | 2 | 12 | Light | 60 s |
Pair it with a pushdown so you cover both the long head and the other two heads. Take two to three seconds to return so you load the stretch. Use the kneeling version if standing is awkward.

Frequently asked questions
Why do overhead extensions work the long head?
The long head crosses the shoulder, so having the arms overhead stretches it and loads it more than with your arms by your sides.
Cable or dumbbell overhead extension?
Cables give constant tension. Dumbbells are easier to find. Both work.
Should I stand or kneel?
Standing uses a staggered stance. Kneeling reduces balance demand and lets you focus on the arms.
How heavy should I go?
Choose a weight where your elbows stay in and your back stays neutral for 10 to 15 reps.
Are overhead extensions bad for the shoulders?
They are fine for most people. If you feel pinching overhead, shorten the range or use another variation.
